How may I obtain my transcript?
Make a request in person or send the High School counselor a written request which includes all of the following information:
1. Your complete name
2. The name you went by in school, if it has changed
3. Your date of birth
4. Social Security number, if you wish the transcript mailed to a college
5. The name of the school you last attended
6. Your complete mailing address and phone number
7. The address you would like your transcript mailed to or the number you want it faxed to
8. Your signature
