Kindergarten Registration
Items needed for registration: birth certificate, copy of parent(s) drivers licenses, social security card, proof of residency, shot record, contact info. (home address, phone numbers, emergency contacts).
Head Start Registration
Items needed for registration: birth certificate, copy of parent(s) drivers licenses, social security card, proof of residency, shot record, contact info. (home address, phone numbers, emergency contacts), proof of income (tax form 1040 pages 1 and 2, W-2(s), 4 most recent pay stubs, self-employment-previous 12 month income:TANF, SSI, Social Security Income, child support, and any public assitance info.
